I fully understand that by signing this commitment to play in the Eastern Hockey Federation (EHF) for the Northeast Generals, my son or daughter is committed to playing for the Northeast Generals for the 2025-2026 hockey season.
FOR GIRLS TEAMS
I fully understand that by signing this commitment to play in the New England Girls Hockey League (NEGHL) for the Northeast Generals, my daughter is committed to playing for the Northeast Generals for the 2025-2026 hockey season.
Release of Liability/Acknowledgment of Risk
In consideration of your child being allowed to participate as a player in any Northeast Generals activity, the undersigned acknowledges and agrees that: (1) The participation in or observation of ice skating, ice hockey and dry land sporting activities have inherent physical risks that may result in serious injury including permanent paralysis or death. (2) I knowingly and freely assume all such risks, both known and unknown, and assume full responsibility for my child’s participation. (3) I understand and agree that neither the Program nor any of its officers, directors, shareholders, employees, agents, coaches, referees, ice rink or employees of the ice rink used by the Program shall be responsible or held liable for any accidents, injury (including paralysis and/or death), loss of equipment or any other costs, expenses, damages, or losses in connection with such participation. (4) The player and their family are responsible for any and all medical costs for any injuries arising from or around the Program activities. (5) I hereby represent to the Program that my son or daughter has no known medical condition that restricts or prohibits participation in ice hockey, ice-skating or any related activity, nor does the player have any known medical condition which puts him or her at greater risk of injury or death resulting from any risks associated with participating in Program activities, whether such risks are known or unknown to parent or player. (6) In the event of injury or illness, I give permission for the Program or its designee to provide, or make arrangements for the provisions of, emergency first aid. (7) The Program reserves the right to terminate the player's participation in activities based upon inappropriate behavior by the player or parent without refund. The player is expected to act in a mature and responsible manner at all times. Any behavior that the Program deems an endangerment to the safety of other persons or property, or that jeopardizes the Program’s ability to conduct its business, will result in the player's immediate expulsion. The player may also be expelled from participation in Program activities for financial delinquency, failure to abide by all Program and ice rink rules and directives and/or falsification of registration information.
PARENT/GUARDIAN’S AGREEMENT TO PAY
Parent/Guardian agrees to pay to the Northeast Generals all of the tuition agreed to in this agreement.
Any and all legal costs involved in collecting the aforementioned tuition will be paid by the parents/guardians. I/we fully understand our financial commitment to Northeast Generals. Both parents/guardians agree to pay full tuition regardless of whether the player should voluntarily leave the team. Under no circumstance will tuition be pro-rated for absences or injury. The Northeast Generals reserves the right to process credit cards (authorization) for any or all tuition payments considered past due on a monthly basis. Failure to pay for tuition in full before 9/1/2025 may result in your player not being permitted to participate in team activities. In addition, the Northeast Generals reserve the right to apply late fees to all accounts considered past due.
Northeast Generals Player Code of Conduct
Players must always represent the Northeast Generals with class and dignity. Players will accept and will not dispute or argue any decision made by an official or coach. Players will show respect to all coaches, teammates, parents, opponents, spectators, facility employees and officials. Players are expected to always display good sportsmanship and never deliberately jeopardize the safety and wellbeing of another individual. Players must strive to be a team player. Obscene or abusive language and inappropriate behavior will not be tolerated in or around the rink. The Northeast Generals will be a program of players that work hard, play hard, and show humility.
Northeast Generals Parent Code of Conduct
The Parent Code of Conduct applies to all direct or related activity that your son or daughter’s team is participating in with the Northeast Generals. It is the parent or guardian's responsibility to behave appropriately and respectfully toward all players, coaches, parents, spectators, officials or employees and property of any facility where events are being held. Failure to comply with this code of conduct may result in expulsion from the Program without a refund.
As a parent/guardian, I pledge to
- Refrain from using obscene or vulgar language in a boisterous manner to anyone at any time.
- Refrain from taunting players, parents, spectators, facility employees, coaches, or officials by means of baiting, ridiculing, criticizing, threatening physical violence, or enacting physical violence.
- Refrain from throwing any object in the spectators’ viewing area, players’ bench, penalty box, locker room, the on-ice surface or anywhere on the premises.
- Fully support and positively reinforce the efforts and feedback of the team coaches. This means refraining from interfering with coaching activities and allowing the coaches do their jobs.
- Fully support and comply with USA Hockey’s Zero Tolerance Policy.
